Take the survey
Welcome! You are invited to take part in one of two surveys exploring how women with chronic pelvic pain, endometriosis and adenomyosis navigate primary care in Aotearoa New Zealand to have their symptoms heard and acted upon.
Taking part is completely up to you and is entirely anonymous. It takes around 5–6 minutes to complete.
Which survey is for you?

What happens next
The survey is anonymous. Because I can’t tell whose answers are whose, I can’t remove your answers once you press submit. If you change your mind before then, just close the survey.
Once you have completed the survey you’ll be taken to a separate page where you can opt in to be considered for stage two of my research, which is an interview. You can also opt in to receive a summary of the findings. That page is not linked to your survey answers, so your answers stay anonymous.
Interviews are optional and come with their own information sheet and consent form.
